Arizona requires anyone registering to vote in the state for the first time to provide proof of citizenship. The easiest way to do this is with your Arizona Driver License or State ID. You simply fill in Box 9 on the form with your license or State ID number.
If you don’t have a license or State ID, there are several ways you can provide proof of citizenship. You can submit a legible copy of one of these documents with the form:
- Birth certificate. (If the name on the certificate is not the same as your current legal name, submit a supporting document like a marriage certificate that explains why they are different)
- U.S. Passport
- Certificate of Naturalization
- Tribal Certificate of Indian Blood
- Tribal or Bureau of Indian Affairs Affidavit of Birth
Or, you can provide this information on the form:
- If you are Native American, you can provide your Bureau of Indian Affairs card number, tribal enrollment number or tribal treaty card number. (Box 10 on the form.)
- If you are a naturalized citizen, you can provide the alien registration number from your Certificate of Naturalization.
Once you have your citizenship information and any supporting documents ready, fill out the one-page Voter Registration Form. Attach a copy the documents your a submtting. Do not attach originals! Then send it to your County Recorder. For Pima County the address is:
